Ethanol/Fuel Tank questions

There has been so many threads and messages on the ethanol/fuel tank issue I am losing track.
Questions;
1) How many people have found the black goo in there engines?
2) How many with actual engine failure?
3) Has anybody successfully made an insurance claim for engine repair and or replacement?
4) Has anybody seen with there own boat any sign of potenial tank failure or actual tank failure?
5) Has anybody seen hose, gasket or internal carburetor deteration?

Have read the lab reports on this page and most comments but I am looking for actual problems and how many.
